Clinical Comparisons of Open and Laparoscopic Appendectomy according to the Type of Acute Appendicitis

Abstract
Purpose: The advantages of the use of a laparoscopic appendectomy for acute appendicitis are still in debate. For a successful laparoscopic appendectomy, the technique of the operator is the most important factor, but also the type of specific appendicitis affects the results of the procedure. The purpose of this study is to evaluate the use of laparoscopic appendectomy based on the type of appendicitis presenting in a patient.

Methods: From July 2006 to June 2007, we enrolled 118 consecutive patients that underwent an appendectomy in our institute. We compared age, incision length, operation time, the number of analgesics administered, hospital stay and rate of postoperative complications between patients that underwent an open appendectomy (OA group) and patients that underwent a laparoscopic appendectomy (LA group). Results for patients in both groups were compared based on the type of acute appendicitis.

Results: There was no significant difference in age, operation time and postoperative complications rate for patients in the two groups. However, incision length, the number of analgesics administered and hospital stay were significantly lower in the LA group of patients. For simple appendicitis, the operation time was significantly shorter in the OA group of patients, but incision length, the number of analgesics administered and hospital stay were lower in the LA group of patients. For complicated appendicitis, there were no significant differences in age, operation time, hospital stay and rate of postoperative complications, but incision length and the number of analgesics administered were significantly lower in the LA group of patients.

Conclusion: In a comparative study reflecting several factors that affect the performance of a laparoscopic appendectomy, the use of laparoscopic surgery has several advantages.
